Project IMPACT: Diabetes Overview Video

This five-minute video provides an overview of Project IMPACT: Diabetes, the first national diabetes self-management program conducted by the American Pharmacists Association (APhA) Foundation in partnership with the Bristol-Myers Squibb Foundation's Together on Diabetes initiative. The hundreds of pharmacists involved in Project IMPACT: Diabetes are working with physicians, nurses, diabetes educators and other members of the health care team to bring quality diabetes care to more than 2000 homeless, poverty stricken, uninsured or under-insured patients with diabetes in 25 communities across the U.S.
