Project IMPACT: Cardiovascular Health+
Through point-of-care encounters and prospective patient profile reviews, pharmacists in this project are identifying patients who could be at-risk for Afib based on medication, medical history, age, and other risk factors.
When a pharmacist identifies an opportunity to engage a patient in conversation regarding his/her cardiovascular health, the pharmacist will explain the Cardiovascular Health+ Service offering, which includes the following:

Screening for social determinants of health (SDoH) that may impact medication use behavior through survey

Completion of a validated stroke risk scorecard

Point-of-care testing with FDA-Cleared EKG technology (KardiaMobile) to screen for Afib and other heart rhythm abnormalities

An assessment of the need for a targeted medication review (TMR) or comprehensive medication review (CMR)
The patient is asked to complete the SDoHRx survey, after which the pharmacist may provide resources and/or refer the patient to an SDoH Case Manager to help identify necessary resources to address social determinants of health. Using the results of the stroke risk scorecard and the point-of-care EKG test, the pharmacist will educate the patient about the risks of Afib and may refer the patient to another provider for further evaluation when necessary. The pharmacists will follow-up with referred patients to inquire about a diagnosis of Afib and/or initiation of treatment, and help them monitor their cardiovascular health.
