Patient Perceptions of and Willingness to Enroll in a Long-Term Care (LTC) at Home Service

Pharmacists play a vital role in helping patients manage medications for chronic conditions within the community pharmacy. This project provides an exciting opportunity to better understand patient perspectives and their willingness to engage in long-term care at home services. By identifying patient needs and readiness, we can strengthen pharmacist involvement in care transitions and enhance the delivery of patient-centered care within the community setting.
My motivation for this project stems from a desire to highlight the expanding role of pharmacists in supporting patients beyond traditional care settings. I am passionate about how pharmacists can optimize medication management, and as healthcare continues to shift toward more home-based models, long-term care at home services can play a crucial role in ensuring patients’ medications are managed safely and effectively. Through this project, I hope to identify ways pharmacists can better support patients in their medication management, ultimately improving their quality of life.
I’m most excited about the opportunity to explore a new area of pharmacy practice with the potential to transform how pharmacists deliver patient care. Receiving an Incentive Grant allows me to engage directly with patients and gain a deeper understanding of their perspectives on pharmacist-led care at home. I look forward to applying these insights to advance innovative, patient-centered models that expand the role of community pharmacists and strengthen continuity of care.
