Implementation and Impact of Pharmacist-Led Diabetes Self-Management Education & Support Services in a Rural, Underserved Community

Our pharmacies and clinics are in various locations in West TN, primarily in rural, underserved areas.


Healthcare access is limited in these areas. Launching a DSMES program will reduce diabetes exacerbations and co-morbidities, which will reduce the healthcare strain on the already limited number of facilities in the area. While on my ambulatory care rotation, I felt very fulfilled when conducting motivational interviews and teaching patients about their diabetes. I enjoy teaching and believe that patients should have knowledge about their health. With that in mind, I wanted to expand the scope of my practice site to include a diabetes self-care program.


Firstly, it is an honor to be selected for this grant and am very appreciative of the funds and the Learning Extension Program that we are being offered. I am most excited to start educating patients on their diabetes and building relationships with patients. I hope that starting this program will serve as a catalyst to start other healthcare programs in the area.
