“I am honored and incredibly grateful to receive an APhA Foundation Incentive Grant this year. This provides the ability to impact patient safety in the community pharmacy setting positively.

Kidney function has a tremendous impact on the majority of medications our patients are on for various disease states. As a community-based pharmacist, I want to ensure my patients are receiving the top level of care, and that includes optimizing medication dosing. This project seeks to highlight the utilization of lab values to ensure medications are being dispensed appropriately to our patients with altered kidney function.

I am excited to see how a simple implementation of obtaining serum creatinine and weight values of our patients in the community-based pharmacy can impact medication dosing. Ultimately, I hope to highlight how this positively impacts patient safety and encourage community pharmacies to obtain this information on their patients as well!
