This project represents our commitment to helping patients overcome real-world barriers to managing their health. In rural communities, challenges like access, cost, and health literacy can make it difficult for patients to stay consistent with their medications. Through programs like PlusPack and Chronic Care Management, we’re creating practical, personalized solutions that make adherence easier and care more meaningful. It’s inspiring to see how pharmacist-led support can not only improve cholesterol management but truly empower patients to take ownership of their health.


I’ve always been passionate about helping patients with chronic conditions feel more empowered to take control of their health. Working in a rural community, I’ve seen how challenges like complex medication routines and limited access to care can make it difficult for patients to stay consistent with treatment. This project grew from a desire to change that—by creating pharmacist-led support systems that simplify care, build trust, and help patients achieve lasting improvements in their health.


I’m most excited about the opportunity this project provides to make a real difference in patients’ lives by improving how they manage their chronic conditions. Receiving an Incentive Grant allows us to expand innovative, pharmacist-led programs that remove barriers to care and promote long-term health. It’s incredibly rewarding to see our efforts translate into better outcomes and stronger connections with the community we serve.
