Exploring the Impact of a Structured Implementation Strategy on Point-of-Care A1C Screening Implementation across the Nashville Division of a Nationwide Community Pharmacy Division

I am truly excited and honored to receive this grant to better improve diabetes management through the expansion of A1c testing services in community pharmacies. Improving how these services are implemented we can reach more patients, identify diabetes earlier, to help patients take better control of their health.
Diabetes continues to be a growing health concern with many patients struggling to receive timely screening, and ongoing management. Community pharmacies are well positioned to help bridge that gap by offering convenient A1c testing and follow-up. This project was motivated by me recognizing the need to support patients but also creating a service that is easier for pharmacy teams to deliver as part of their everyday workflow.
I am excited to help pharmacy teams feel supported and confident in providing A1c testing, while also helping patients better manage their diabetes. This project allows me to use implementation strategies that make delivering clinical services easier and more sustainable to deliver. I look forward to seeing how these changes can improve the health of patients and the pharmacy’s role in chronic disease management.
