"The motivation behind my project stems from a growing awareness of the critical role that social determinants of health play in shaping an individual’s health and well-being. By examining these health disparities and barriers, my project seeks to enhance the quality of pharmacist-provided nutrition counseling, which ultimately improves the health outcomes of those within the community, especially those with a chronic disease state. Overall, the project seeks to ensure that individuals, regardless of their socioeconomic background or life circumstances, can access the nutritional support they need to achieve optimal health."
