Pharmacist-Driven Remote Patient Continuous Glucose Monitoring for Medicaid Patients

I am incredibly honored to receive an Incentive Grant to advance pharmacist-driven remote continuous glucose monitoring (CGM) for Medicaid patients. This project aims to expand access to innovative diabetes care through pharmacist-led interventions, improving outcomes for patients who often face barriers to consistent glucose monitoring. By integrating remote CGM into clinical practice, I hope to demonstrate the vital role pharmacists play in optimizing diabetes management and bridging gaps in equitable healthcare access.


My motivation for this project stems from witnessing the disparities in diabetes care among underserved populations, particularly Medicaid patients who face financial, technological, and systemic barriers to continuous glucose monitoring. Pharmacists are uniquely positioned to provide accessible, patient-centered care that can transform diabetes management. Through this project, I aim to highlight how pharmacist-led CGM programs can improve glycemic control, reduce complications, and empower patients to take charge of their health.


What excites me most about this project is the opportunity to bring innovative diabetes technology to patients who need it most and to demonstrate the tangible impact pharmacists can have on chronic disease management. I am eager to see how increased access to CGM can improve patient engagement, enhance clinical outcomes, and promote health equity. Receiving this grant allows me to contribute meaningful research that can inspire future pharmacist-led initiatives nationwide.
