“Social determinants of health (SDoH) are the environmental conditions where people are born, live, learn, work, play, worship, and age. SDoH has a major impact on people’s overall health, well-being, and quality of life. Currently, there is no universally accepted SDoH screening tool across all healthcare settings. Community pharmacists have a unique opportunity to integrate SDoH screening into their practices and offer enhanced services.

The motivation behind this project is to demonstrate community pharmacists' ability to address both health and social-related needs for all patients.

I'm excited about this project because it showcases the wide range of services community pharmacists offer to support patients holistically. Our pharmacy teams will be able to provide resources through various methods, including text messaging, among others.”
