Effectiveness of Pharmacist-Integrated Team-Based Care Versus Primary Care Alone in an Onsite Employee Health Center

Pharmacists are integral to the healthcare team and have proven time and time again how influential they can be on patient care. This project aims to reenforce that idea in an employer-based healthcare clinic by evaluating the differences in clinical outcomes and medication adherence between patients receiving pharmacist integrated team-based care and those receiving primary care alone.


My motivation for this project stems from a desire to demonstrate the meaningful impact pharmacists are making when integrated into team-based care, particularly for patients with chronic conditions like diabetes. My goal is to showcase the value that pharmacists bring to improving patient outcomes and supporting the healthcare team as a whole. I’m passionate about empowering patients to take control of their health and showing how pharmacists can be a key part of that journey.


I am most excited to see how the results of this project highlight the positive impact pharmacists can have when working alongside primary care providers to help patients manage their diabetes. The Incentive Grant helps to provide the resources needed to complete meaningful research that can help advance the role of pharmacists in collaborative care settings.
