" When it came time to choosing a project, I wanted to choose one that would be meaningful. Uncontrolled hypertension is a concern for many patients seen at our community health center. I hope that this project will make an impact in patient care by improving access to home blood pressure monitors and integrating education into patient visits."
"I am most excited to integrate the distribution of home blood pressure monitors into our initiative. In addition to providing monitors to our patients, we will be providing patient counseling. Pharmacists are uniquely positioned to intervene and provide individual education and counseling to our patients."
