2023 Incentive Grant Recipient: Justin Han, PharmD

 

 

Project Proposal: Incorporating Social Determinants of Health into Workflows Within a Specialty Pharmacy

Justin Han, PharmD currently serves as the PGY1 Community-based Pharmacy Resident at the University of North Carolina Eshelman School of Pharmacy in Chapel Hill, North Carolina and the UNC Health Specialty and Home Delivery Pharmacy in Durham, North Carolina.

"Increasingly we are seeing that social determinants of health play a significant role in affecting the health of patients. Pharmacists are incredibly accessible health-care providers and specialty pharmacies are adept in helping patients navigate their treatment journeys across various complex disease states. I believe his makes specialty pharmacies well-positioned to identify and address social determinants of health issues while providing high-quality patient care. "
