Project Proposal: Incorporating Social Determinants of Health into Workflows Within a Specialty Pharmacy
Justin Han, PharmD currently serves as the PGY1 Community-based Pharmacy Resident at the University of North Carolina Eshelman School of Pharmacy in Chapel Hill, North Carolina and the UNC Health Specialty and Home Delivery Pharmacy in Durham, North Carolina.

Pharmacists are incredibly accessible health-care providers and specialty pharmacies are adept in helping patients navigate their treatment journeys across various complex disease states.
