2024 Incentive Grant Recipient: Jennifer Tram Nguyen, PharmD

 

Project Proposal: Assessment of the Impact of Social Determinants of Health Barriers on Community Pharmacist-Provided Nutrition Counseling

Jennifer Tram Nguyen is a PGY1 Community-based Pharmacy Resident with Albertsons Companies in Boise, ID. She graduated from American University of Health Sciences. 

"Community pharmacists have evolved beyond traditional roles and have become pivotal figures in promoting patient-centered healthcare."

"The motivation behind my project stems from a growing awareness of the critical role that social determinants of health play in shaping an individual’s health and well-being. By examining these health disparities and barriers, my project seeks to enhance the quality of pharmacist-provided nutrition counseling, which ultimately improves the health outcomes of those within the community, especially those with a chronic disease state. Overall, the project seeks to ensure that individuals, regardless of their socioeconomic background or life circumstances, can access the nutritional support they need to achieve optimal health."

"I am honored to be a recipient of this year’s Incentive Grant. I am excited to be given the opportunity to better explore and address the often-overlooked social determinants of health regarding nutrition.
