2023 Incentive Grant Recipient: Durdana Iqbal, PharmD

 

 

Project Proposal: Perceptions of Attitudes of Social Determinants of Health (SDoH) Program Champions within CPESN Community Pharmacies

Durdana Iqbal, PharmD, currently serves as the Clinical Research Fellow for Pharmaceutical Health Services and Outcomes at the University at Buffalo in Buffalo, NY.

"Social determinants of health (SDoH) are an important aspect of public health and pharmacies are uniquely positioned to address patient’s social needs through screening and referral programs. CPESN NY will be implementing a SDoH Pharmacy Program, and my project will assess perceptions of project champions to better understand program implementation and for continued development."

 "Social needs screening and referral programs within community pharmacies are relatively new and there is limited knowledge on what are the best practices for implementation. Project Champions’ feedback is essential to identify barriers and facilitators of the program in order to make effective, long-lasting improvements. My motivation for this project was the opportunity to work with pharmacies, staff and the program champions, so that we can continue to improve the program and ultimately expand it throughout New York State."
