Project Proposal: Assessment of a Community Pharmacist Remote Monitoring Service in Patients Using Continuous Glucose Monitors (CGMs)

Cody Beldon, PharmD currently serves as a PGY1 Community-based Pharmacy Resident at Kroger Health/University of Cincinnati College of Pharmacy in Cincinnati, Ohio

“Community pharmacists are the most accessible healthcare professional, providing high-quality patient care services. This project will incorporate continuous glucose monitoring within a community pharmacy setting to allow for enhanced, collaborative diabetes care management.”

"A CGM remote monitoring service combines two of my passions: diabetes education and technology. More and more patients have begun to utilize CGMs to improve their diabetes control, and I have witnessed the need for closer diabetes monitoring while working with some of these patients at my pharmacy. I have a close family member who currently uses a CGM, and I have seen the improvements in his diabetes care from the data collected from the CGM cloud-based platform."

"This study will be one of the first CGM remote monitoring studies completed in a community pharmacy. I am beyond excited to share this new, individualized service with our patients, provide high-quality comprehensive diabetes education, and prove that community pharmacists can provide another valuable service as a key player on the healthcare team."
