2022 Incentive Grant Recipient: Ashley Grubbs Eason, PharmD

 

 

Project Proposal: Impact of Pharmacist-Led Cardiovascular Health Monitoring and Education on ASCVD Risk Score in the Community Pharmacy Setting

Ashley Grubbs Eason, PharmD currently serves as the PGY-1 Community Pharmacy Resident at Josefs Pharmacy/Campbell University in Raleigh/Buies Creek, NC. She is a graduate of Campbell University

The community pharmacy world is rapidly changing and there are so many opportunities for pharmacists to step in to provide education, manage medication therapy, and provide clinical services. The goal of this program is to identify patients who may not be regularly receiving appropriate care or may not have received adequate education or follow-up regarding their cardiovascular health. Ashley's hopes upon completion of this project is to show the impact of education and interventions made by pharmacists on patients’ overall health.

"I am grateful to be a recipient of an Incentive Grant for the 2021 – 2022 year which will allow me the opportunity to provide patient care services within the community setting. From these patient care services I will be able to provide education, remove adherence barriers, and make primary care provider referrals for patients to improve their disease state knowledge and care."
