Diabetes Patient and Project IMPACT: Diabetes Participant

Bonnie is a patient engaged in one of the Foundation’s Project IMPACT: Diabetes community sites, a federally qualified health center in Virginia.  Bonnie is dependent on shelter housing and lives with type 2 diabetes.  Before receiving services through Project IMPACT, Bonnie did not know she had diabetes, let alone how to manage her chronic illness.  With the help of pharmacists participating in this program through the APhA Foundation, Bonnie and many others who are uninsured, homeless, or at risk of homelessness are receiving the care and education they need to maintain a healthy lifestyle.

Through Project IMPACT: Diabetes, Bonnie and other patients are receiving:

  • Regular visits with pharmacists, often in conjunction with physicians and dietitians
  • Glucose testing supplies
  • Patient self-management assessments
  • Customized one-on-one diabetes education
  • Ongoing medication therapy management to help monitor adherence, lifestyle choices and other health issues they might have
