About

Align My Refills is a public education initiative of the American Pharmacists Association (APhA) Foundation, designed to inform patients and caregivers on how to engage with pharmacists through medication synchronization programs to improve medication use and achieve better health outcomes. The American Pharmacists Association (APhA) Foundation, a not for profit 501(c)(3) organization based in Washington, D.C., is a trusted source of research demonstrating how pharmacists can improve health care. The APhA Foundation’s mission is to improve people’s health through pharmacists’ patient care services.

National Council on Patient Information and Education (NCPIE)

NCPIE is a lead partner on the Align My Refills campaign, which launches during NCPIE's 29th Annual "Talk About Your Medicines" Month with a theme this year of "Let's Get in Sync." NCPIE is one of the original patient safety coalitions and has been working to advance the safe, appropriate use of medicines through enhanced communication since 1982. NCPIE's Adherence Action Agenda (A3) Project promotes safe and appropriate medicine.

CAREGIVER ACTION NETWORK

Caregiver Action Network is the nation’s leading family caregiver organization working to improve the quality of life for the than 65 million Americans who care for loved ones with chronic conditions, disabilities, disease, or the frailties of old age. CAN serves a broad spectrum of family caregivers.

Men's Health Network

Men's Health Network (MHN) is a national non-profit organization whose mission is to reach men, boys, and their families where they live, work, play, and pray with health awareness messages and tools, screening programs, educational materials, advocacy opportunities, and patient navigation.

National Alliance for Caregiving

Established in 1996, The National Alliance for Caregiving is a non-profit coalition of national organizations focusing on advancing family caregiving through research, innovation and advocacy. Alliance members include grassroots organizations, professional associations, service organizations, disease-specific organizations, government agencies, and corporations.

National Osteoporosis Foundation

The National Osteoporosis Foundation (NOF) is the leading health organization dedicated to preventing osteoporosis and broken bones, promoting strong bones for life and reducing human suffering through programs of public and clinician awareness, education, advocacy and research. Established in 1984, NOF is the nation's only health organization solely dedicated to osteoporosis and bone health.

Script Your Future (National Consumers League)

Script Your Future is a national public education campaign developed to raise awareness among patients, their family caregivers and health care professionals, about the importance of taking medications as directed. The campaign, led by the National Consumers League, focuses on patients affected by diabetes, respiratory disease, and cardiovascular disease. The interactive Script Your Future website offers tools for both patients and health care professionals to help improve adherence.
