Project Proposal: Community Pharmacist-Led Intervention to Identify Statin Eligible Persons

“I hope to make a positive impact on my patient’s lives with my screening tool. I believe that this tool will help identify patients who are at high risk for cardiovascular disease and prevent the possibility of future negative cardiovascular outcomes.”

Nicole Jara, PharmD, currently serves as the PGY1 Community Pharmacy Practice Resident at Atlantis Pharmacy & Palm Beach Atlantic University in Atlantis, FL.

Dr. Jara hopes to develop and implement a Statin Eligibility Screening Tool to identify statin eligible patients categorized into one of four Statin Benefit Groups.
