Project Proposal: Use of a Health Risk Screening to Influence Enrollment in the CDC Diabetes Prevention Program at Balls Food Stores

“Many of my family members and some of those close to me have been affected by diabetes.”

Dr. Snodgrass is a PGY1 community pharmacy resident with the University of Missouri-Kansas City School of Pharmacy and Balls Food Stores in Gladstone, MO. Of the 84.1 million Americans with prediabetes, a Centers for Disease Control (CDC) report revealed only 11.6% of these adults are aware of this status. The CDC created a Diabetes Prevention Program (DPP), which this project aims to expand. 

Chelsea Snodgrass is excited to use the Incentive Grant funds to screen patients and “will hopefully lead to increased awareness of risk, and ultimately, increased enrollment in our program.”
