2023 Incentive Grant Recipient: Rafael Sánchez, PharmD

 

 

Project Proposal: Optimizing HEARTS (Heart Failure Education, Assessment, and Referrals in The Community Pharmacy Setting)

Born and raised in West Palm Beach, Florida, Dr. Sánchez completed his undergraduate studies at the University of North Carolina-Chapel Hill and earned his Doctor of Pharmacy from the University of Florida’s College of Pharmacy. He recently completed his post-graduate year 2 (PGY2) specialized residency training in ambulatory care at UF Health in Gainesville, Florida. Currently, he practices as a clinical pharmacy practitioner at a Federally Qualified Health Center (FQHC) named “Centro de Salud Familiar – La Fe,” and as a Clinical Assistant Professor at the University of Texas (UTEP) School of Pharmacy.

His professional interests include family medicine/primary care, HIV prevention and management, diabetes technology and academia. Eventually, he plans to obtain a master's in health administration to gain proficiency in financial and strategic decision-making to succeed in a pharmacy leadership position.

Outside of pharmacy, Dr. Sánchez enjoys exploring local breweries, sketching, playing piano, and spending time with his Shiba Inu, Jonah, and French bulldog, Lucas.
