Aspects of Implementation

Settings

Ambulatory care settings will be the primary locations for APhA Foundation research project implementation, with a specific focus on care delivery within:

  • Accountable Care Organizations (ACOs)
  • Community Pharmacies
  • Patient Centered Medical Homes (PCMHs)
  • Pharmacy-based Clinics
  • Primary Care Provider Office Practices

Collaboration with Healthcare Team Members

Collaboration among members of the healthcare team is essential for effective patient care. The APhA Foundation’s research initiatives will include all willing members of the local healthcare teams, especially:

  • Advance Practice Nurses
  • Dieticians and Nutritionists
  • Physicians
  • Physician Assistants
  • Social Workers

Patient Populations

All patients should have access to pharmacists’ patient care services. In an effort to generate evidence for on populations that are in highest need of pharmacists’ services, the APhA Foundation’s priority populations will include:

  • ACO Beneficiaries
  • Medicaid Beneficiaries
  • Medicare Beneficiaries
  • Self-insured Employer Beneficiaries 
  • Uninsured or Underinsured
