Project Proposal: Evaluation of a Revised Community Pharmacist-led Transitions of Care Service at a Community Hospital
Brett Becker, PharmD, currently serves as the PGY1 Community Pharmacy Resident at MercyOne Medical Center in Dubuque, Iowa.
Transitions of care is a vulnerable time for patients and a time where community pharmacists can make valuable interventions. Community pharmacists are often underutilized during care transitions and MercyOne-Dubuque strives to use community pharmacists to their fullest potential. By doing so, we hope to have a positive impact on patient care .
I am most excited for the potential of creating a sustainable service that will benefit my site and, more importantly, the members of this community. I also hope to contribute to the growing literature on community pharmacists’ role during transitions of care. I am honored to receive an Incentive Grant for my project and grateful for the opportunity to work for an organization that continually finds ways to improve patient care through innovative services in community pharmacy.
