Project Proposal: Impact of a Community Pharmacy-Based Initiative to Increase Patients Ability to Recognize Heart Failure Symptoms and Improve Self-Management

Samuel Pierini, Pharm.D. currently serves as the PGY1 Community Pharmacy Resident at Albertsons Companies, affiliated with Safeway Pharmacy and Oregon State University in Portland, Oregon. Dr. Pierini graduated from the University of Wisconsin-Madison.

Heart failure continues to be one of the leading causes of emergency room visits and hospital readmissions in the United States. Community pharmacists play a key role in disease state management and are positioned to make self-care interventions, improve adherence and educate patients to decrease hospital visits and improve patient outcomes
