2023 Incentive Grant Recipient: Tatiana Pleil-Pino, PharmD

 

 

Project Proposal: Optimizing HEARTS (Heart Failure Education, Assessment, and Referrals in The Community Pharmacy Setting)

Tatiana Pleil-Pino, PharmD currently serves as the PGY1 Community Pharmacy Resident at Centro de Salud Familiar La Fe Inc and the University of Texas at El Paso in El Paso, Texas 

"The motivation behind this proposal came from my passion for cardiology including heart failure. Underserved populations have limited access to resources and healthcare and are more likely not on optimized medication therapy. Community pharmacists are the most accessible healthcare provider and therefore can make a large impact and bridge the gaps in care for patients with heart failure in an underserved community."

 "I am incredibly grateful to be a recipient of this Incentive Grant. I am looking forward to the opportunity to identify how a community pharmacist can bridge the gaps in care that patients with heart failure face. The pharmacy services implemented will allow me to provide education, remove access barriers and provide referrals for patients to optimize their medication therapy."
