Project Proposal: Impact of Pharmacist Education Visits on Hypertension for an Underserved Population
“Pharmacists are in a good position to provide disease-specific education to patients and providers and play important roles in collaborative care teams to help manage patients' chronic conditions.”
Dr. Kadakia is a PGY1 community pharmacy resident at the University of Cincinnati James L. Winkle College of Pharmacy and Cincinnati Health Department. The subject of her proposal, hypertension, affects about one in three American adults and the negative implications of this chronic condition include an increased risk of heart attack or stroke. Dr. Kadakia is motivated to see how monthly visits with a pharmacist for hypertension health education impacts patient's’ blood pressure control. The topics highlighted in her project include disease state and medication education, therapeutic lifestyle changes, and stress management.
Nira Kadakia is most excited about "being able to provide resources, such as at-home blood pressure monitors, to patients so they can see the impact of their medications and lifestyle changes on their blood pressure control without having to go to a doctor's office or pharmacy!"
